Your session is entirely focused on you. As a licensed Doctor of Physical Therapy, I provide direct 1:1 attention to ensure the highest movement quality and peak symptom response, helping you get back to full activities faster.
Insurance Freedom
Choice Rehab Performance is an out-of-network provider, meaning your care is never dictated by an insurance company. We accept HSA/FSA cards and offer superbills so you can seek reimbursement directly for your sessions.
No Hidden Fees
Expect total transparency in your recovery journey. We provide flat-rate pricing with no surprise bills months later. We offer flexible payment plans depending on your circumstances to ensure elite care is within reach.
How Payment Works
- Cash-Pay Excellence: We don't bill insurance directly, allowing us to focus entirely on your needs rather than network restrictions.
- FSA/HSA Accepted: You can use your Health Savings or Flexible Spending accounts.
- Transparency: A 3% card fee applies via the card vendor, not Choice Rehab Performance.
- Reimbursement: We can provide a superbill for you to submit to your insurance for potential out-of-network reimbursement.
- Flexible Plans: Payment plans are available depending on individual circumstances.
